Eden Supportive Living offers Supportive Living Facilities (SLFs) as an alternative to nursing home care for low-income persons with physical disabilities (22 and older) under Medicaid. The program is designed to preserve privacy and autonomy while emphasizing health and wellness for persons otherwise needing nursing facility care. Eden focuses specifically on people living with disabilities such as Multiple Sclerosis, TBI, stroke, kidney failure, spinal cord injury, kidney failure, and more. The facilities can provide recuperative and preventative nursing care, 24-hour response/security, meals, socialization, and more.Please contact individual communities for more information about eligibility and availability.

The season begins in May and runs through the end of September. Disabled residents of a single-family home responsible for the lawn maintenance of said property and are unable to perform such obligation may be eligible for this program.
