Provides individualized support to individuals with intellectual and developmental disabilities in the comfort of their own homes. Services include developing customized plans that address individual needs and goals, personal care assistance, therapy services (including physical, occupational, and speech therapy), behavioral support, mobility modifications, home modifications, and/or life skills training.

Senior center with approximately 6,000 square feet devoted exclusively to senior citizens and senior groups in the community. The space includes a Senior Social Hall, a separate space for individual activities, and one for group activities.

Offers seasonal sports teams to youth, including basketball, softball, chess, judo, and more.

Outreach staff visit older persons or their caregivers to inform them about available benefits and services. Outreach Services may include:

- Encouraging older persons to participate in senior programs.

- Assisting older persons in gaining access to needed services (Benefit Access Application, Medicare Part D, etc.).

- Follow up with older persons and agencies to determine whether services have been received and the needs of the older person are met.

- Provide client advocacy to secure benefits.

Provides health care services to those in need. Services include sick care, chronic disease management, preventive care services, cervical cancer screenings (Pap smears), physical  exams, vaccinations, sexual health services, STI testing and treatment, wellness checkups, work physicals, laboratory and diagnostic testing, referrals for mammograms and other specialty care.

Provides a dual-diagnosis/co-occurring mental health and substance abuse treatment facility. Services include treatment to those with substance use disorder, with or without a mental health diagnosis. Substance use treatment options vary by location, but may include residential treatment, outpatient treatment, virtual addiction treatment, and detoxification.

Also provides medication-assisted treatment (MAT) services.

Offers to educate businesses, the public, and legislature about issues important to people with disabilities. Staff aim to empower people with disabilities to assess their needs, identify potential risks, establish their own goals, evaluate available resources, and develop and implement a strategy to take control of their lives.
