Offers supportive care. Patients may receive visits or phone calls from a nurse practitioner, a nurse, or a social worker. Supportive care offers visits at FHN offices and at FHN Memorial Hospital, in long-term care facilities, and in the home for those unable to travel to the clinic due to physical disabilities or distressing symptoms.

Offers guidance in forming safety plans, temporary emergency safe shelter, and transportation when needed. Operates hotlines that provide immediate assistance, emotional support, and information for individuals experiencing dating abuse, domestic violence, or sexual assault.

Hotline staff are accessible via telephone, email, chat, and text to ensure safety, provide referrals for medical, legal, and emotional needs, and offer advocacy and other related services. Additionally, youth and adult education programs are available with age-appropriate presentations on consent, body autonomy, sexual harassment, and bullying.

Offers short-term counseling to seniors in their home (McLean County only) or through a virtual platform (all other counties). The program focuses on positive behavior activation to break the depression cycle. Services include a case manager who will monitor individuals weekly to ensure they are linked with the necessary resources and referrals for both inpatient and outpatient drug treatment programs. Services may also include weekly in-home and out-of-home counseling for adults, children, couples, and families. Families may also be referred to a number of community-based services, including medical services, counseling, drug or alcohol treatment support groups, food pantries, housing, employment training, and continuing education programs.
