Provides temporary financial assistance for veterans and their families experiencing a financial emergency. Assistance may be paid towards housing costs, transportation expenses, utilities, and food and fuel certificates. Transportation to VA medical appointments is also available.
Housing Opportunities and Maintenance for the Elderly (H.O.M.E) works with professional moving companies to provide subsidized moving service to qualifying seniors moving within the City of Chicago. The move coordinator will first visit the client to conduct an income assessment and a moving assessment to determine the type of services that are needed (hoarding, general move, etc.). The rest of the move will be completed by the moving company.
SSVF is a Veterans Affairs funded program which is delivered through Primary Health Care. This program is focused on helping homeless and unstably-housed Veterans and their families living in Warren, Polk, Dallas, Jasper, Story, and Marshall Counties. Services provided include:

CASE MANAGEMENT services that will assist participants in obtaining VA and other public benefits, including: health care referrals, daily living referrals, personal financial planning, fiduciary and payee referrals, legal services, housing counseling services, employment and training referrals.

TEMPORARY FINANCIAL ASSISTANCE (TFA) which may include rent assistance, moving expenses, security and utility deposits, transportation, and child care. All financial services based on eligibility and available funding.

Provides consultation and assists seniors moving out of their homes to an assisted living or nursing facilities.

Creates a custom checklist for moving preparations. Visit the website to fill out a form with information about the tasks needed to complete and how to complete them. Each checklist is tailored to the unique needs of each individual or family. Created lists can be saved, download and printed at any time during the moving process and use for future moves.

Provides moving and storage services to Defense Department and Coast Guard personnel and their families. Individuals and families can browse resources, fact sheets, and customer service pages to prepare and implement upcoming moves.

Funding provided by the VA to improve the housing stability of low income Veterans. May be able to assist in moving expense assistance for homeless Veterans in need.

Cross Outreach is a homelessness prevention program providing limited assistance with basic needs such as rent, security deposits, electric or water bills, laundry vouchers or storage costs for households in transition as funding is available. Does not assist with car gas.

***Rent, security deposits, electric, water and laundry voucher assistance requires clients to email a help request on the 3rd Monday of every month between 8 am-8 pm.

2025 Intake Dates are as follows (3rd Monday of each month, 8 am-8 pm): Apr 21, May 19, Jun 16, Jul 21, Aug 18, Sep 15, Oct 20, Nov 17 and Dec 15, 2025.
