CEDA - Department of Family Support and Community Engagement (FSACE) - West Region provides short-term case management services to families seeking financial assistance through other FSACE programs. Case management includes an assessment of family needs and assists in the development of the Family Action Plan, information and referral, financial literacy, nutrition education and enrollment in other FSACE, agency, and community programs. Clients may be eligible for financial assistance to address other barriers. Any financial assistance will be paid directly to the approved provider.
Heartland Alliance - James West Health Center provides case management, housing support, and linkage to services for individuals who are homeless, with a serious mental illness and who may have a co-occurring substance use disorder.
Goodwill Industries of Metropolitan Chicago - Goodwill Workforce Connection Center - Englewood offers comprehensive case management services to individuals who are deaf or hard of hearing. Clients work with a staff that is fluent in American Sign Language (ASL) through individual one-on-one employment counseling, vocational training, job placement, and job coaching to ensure successful employment and job retention.
Polish American Association's (PAA) Immigrant Family Resource Program is a partnership between ICIRR (Illinois Coalition for Immigrants and Refugees Rights), IDHS (Illinois Department of Human Services), and CBO (Community-Based Organizations). The purpose of is program is to inform those in the Polish community with limited English skills about services and eligibility requirements of IDHS programs. PAA offers it's clients:Bi-lingual case counseling services through one-on-one meetings with immigrants and/or immigrant families to determine their needsAssist clients in applying for/accessing appropriate programsMediate with IDHS as needed to insure receipt and maintenance of IDHS-administered benefitsProvide referrals and assistance with accessing other safety net programs for persons not eligible for IDHS-administered benefitsAssist local IDHS offices with interpretation for their Polish-speaking clients
Casa Esperanza Project offers weekly case management services to our adult residents. This service assists residents with goal setting and resources, including enrollment into school/vocational programs, employment, medical care, and housing.

Saint Anthony operates a community wellness program out of several locations. Apostolic Pentecostal Church of Morgan Park Inc. Community Healing Resource Center provides holistic healing services and trauma-informed care to people impacted by gun violence and other forms of violence, directly or indirectly, free of charge. The model not only serves victims of violent crimes and their families but the entire community as well. They can provide: 1) Trauma-Informed Clinical Case Management: Assistance accessing victim services and violence reimbursement application funds, crisis intervention, service coordination, safety planning, and self-care guidance. 2) Outpatient Clinical Counseling and Medication Management: Individual and family counseling services for adults and children. (Some services require insurance and/or Medicaid/Medicare). 3) Community Outreach and Education: Community education for trauma-informed care, safety and self-care guidance, and resiliency responses to trauma and violence. 4) Community Healing Advocacy: Public speakers bureau, legal advocacy, community memorials, and pop-up peace events.

The Salvation Army's Pathway of Hope case management program helps families work their way out of poverty. Their case workers provide comprehensive intake interviews and help identify public benefits assistance and programs that clients are eligible for. Public programs this applies to include SNAP, TANF, WIC, employment assistance, LIHEAP/CEDA, daycare, Affordable Healthcare Act, translation/interpretation services, gas and electricity, and Social Security.

Saint Leonard's Ministries provides supportive transitional housing for formerly incarcerated men and women through Saint Leonard's House, Grace House (women only), Saint Andrew's Court, and Harvest Commons Residence. This includes case management, benefits application assistance, AA groups, NA groups, relapse prevention, spiritual enrichment, and permanent housing search assistance.
