The Community Healing Resource Center provides holistic healing services and trauma-informed care to people impacted by gun violence and other forms of violence, directly or indirectly. At CTJC, Case Management consists of holistic reentry services for survivors as they rebuild their lives in the wake of the sustained trauma of police violence and incarceration. These supports include assistance in securing housing, food, medical insurance, employment and educational resources, emergency financial assistance, and bail bonds. They also engage in crisis care, court accompaniment, transportation assistance, and driver's license acquisition.

Franciscan Outreach case managers conduct initial assessments and provide guidance to individuals experiencing homelessness. Case managers also provide individuals referrals to the following services: ID documents, Social Security benefits, public aid resources, veterans benefits, primary and mental health services, substance use disorder (SUD) treatment, legal aid, job training programs, employment opportunities, immigration services, and housing programs. This location also services as an in-person Coordinated Entry System access point.Please note that case management at the Pilsen location is only offered to current guests of that shelter.
