The Proactive Community Services (PCS) care coordination model utilizes an interdisciplinary team-based client approach. They combine primary care, medical case management, housing navigation/technician, ADAP (AIDS Drug Assistance Program) 340B prescription service with pharmacy assistance and medical adherence, and community health outreach with structured health education to promote HIV care continuum engagement and viral suppression.

Case managers provide guidance through medical insurance, financial assistance, adherence to medical appointments and medications, behavioral health and substance use, oral health, nutrition, transportation, and other basic needs on an ongoing basis.
Franciscan Outreach case managers conduct initial assessments and provide guidance to individuals experiencing homelessness. Case managers also provide individuals referrals to the following services: ID documents, Social Security benefits, public aid resources, veterans benefits, primary and mental health services, substance use disorder (SUD) treatment, legal aid, job training programs, employment opportunities, immigration services, and housing programs. This location also services as an in-person Coordinated Entry System access point.Please note that case management at the Pilsen location is only offered to current guests of that shelter.
