As individuals age, navigating the intricacies of the medical system becomes increasingly difficult. Their knowledgeable care managers develop long and short-term plans allowing seniors to experience the highest quality of life possible.

Midwest Care Management Services provides experienced and compassionate Care Managers to create a plan to address your health, safety, daily caregiving, and social needs. They serve as a liaison between invested parties and find the best nursing home, assisted living, or supportive arrangements. Care Managers advocate for the best care and treatment at hospitals and facilities along with coordinating end-of-life planning and logistics.

Franciscan Outreach case managers conduct initial assessments and provide guidance to individuals experiencing homelessness. Case managers also provide individuals referrals to the following services: ID documents, Social Security benefits, public aid resources, veterans benefits, primary and mental health services, substance use disorder (SUD) treatment, legal aid, job training programs, employment opportunities, immigration services, and housing programs.Please note that case management is offered at the Pilsen location only for current guests of that shelter.

GreenBax Health Solutions provides chronic illness care/management to those with diabetes, high blood pressure, COPD, and other conditions. They help manage those conditions whether it be medication management, lifestyle modifications or other management that improves the health and lives of patients.
Apostolic Pentecostal Church of Morgan Park Inc. Community Healing Resource Center provides holistic healing services and trauma-informed care to people impacted by gun violence and other forms of violence, directly or indirectly, free of charge. The model not only serves victims of violent crimes and their families but the entire community as well. They can provide: 1) Trauma-Informed Clinical Case Management: Assistance accessing victim services and violence reimbursement application funds, crisis intervention, service coordination, safety planning, and self-care guidance. 2) Outpatient Clinical Counseling and Medication Management: Individual and family counseling services for adults and children. (Some services require insurance and/or Medicaid/Medicare). 3) Community Outreach and Education: Community education for trauma-informed care, safety and self-care guidance, and resiliency responses to trauma and violence. 4) Community Healing Advocacy: Public speakers bureau, legal advocacy, community memorials, and pop-up peace events.

Marillac St. Vincent Family Services' Intensive Case Advocacy & Support (ICAS) Program provides comprehensive supportive services to at-risk, self-neglectful seniors throughout Chicago to ensure safety, stability, and the ability to successfully age in place.
CTF Illinois delivers case management services to over 500 individuals within its residential, vocational, and home-based programs. Case Managers are responsible for advocating for the support and care an individual receives by focusing on the individual's choices and goals. Case Managers link every person served to an individualized medical, therapeutic, social, and educational-based service plan.
Transforming Re-Entry Services (TRS) offers a variety of health and wellness programs including health promotion services, health screenings, substance use disorder services, behavioral health counseling by referral, harm reduction, and HIV/HCV case management.

The Proactive Community Services (PCS) care coordination model utilizes an interdisciplinary team-based client approach. They combine primary care, medical case management, housing navigation/technician, ADAP (AIDS Drug Assistance Program) 340B prescription service with pharmacy assistance and medical adherence, and community health outreach with structured health education to promote HIV care continuum engagement and viral suppression.
At CTJC, Case Management consists of holistic reentry services for survivors as they rebuild their lives in the wake of the sustained trauma of police violence and incarceration. These supports include assistance in securing housing, food, medical insurance, employment and educational resources, emergency financial assistance, and bail bonds. They also engage in crisis care, court accompaniment, transportation assistance, and driver's license acquisition.
The Open Doors Program invites adults to engage in year-long programming, featuring weekly small-group meetings, one-to-one case management, guest speakers, and field trips.
