Franciscan Outreach case managers conduct initial assessments and guide individuals experiencing homelessness. Case managers also provide referrals to the following services: ID documents, Social Security benefits, public aid resources, veterans' benefits, primary and mental health services, substance use disorder (SUD) treatment, legal aid, job training programs, employment opportunities, immigration services, and housing programs. This location also serves as an in-person access point to the Coordinated Entry System.Please note that case management at the Pilsen location is available only to current guests of that shelter.
