Services include annual screenings or check ups, chronic disease management, immunizations, pregnancy tests, pre and postnatal care, school and sports physicals, sexually transmitted infection tests, and well-child exams. 

Provides assistance to formerly incarcerated individuals and their families with wraparound services and case management focusing on a 72-hour transitional plan, employment, and individualized coaching support. Also offers counseling, cognitive-behavioral therapy, anger management, parole/probation requirements, transportation, substance evaluation referrals.

Provides programs for youth that promote health, social, educational, cultural, leadership and character development.  Youth served are identified as at-risk based on assessments completed that identify such factors as economic level, family status, neighborhood location, and academic progress.

Offers a supplemental food and nutritional counseling service. All participants receive nutrition education and counseling, supplemental foods, and information about other health services based on assessment. A breastfeeding peer counselor is available to clients who need breastfeeding support.

Habitat carries a 0% interest mortgage on the home for a period of 15-20 years until the partner family owns their home free and clear.

Provides education and support services to students. Services aim to help students acquire an Illinois high school diploma, formerly known as a high school equivalency certificate, and acquire English language skills for English language learners by offering language acquisition (ELA) classes. Will also help students with the transition into college or a career pathway by offering career readiness curriculum in HSE and ELA classes and assistance transitioning into on-the-job training, college, and careers.

The program also offers nutrition education, which includes guidance on healthful eating, food handling, preparation, shopping skills, and the relationship between nutrition and overall health. Information and counseling about Affordable Care Act (ACA) insurance options are available, including assistance with enrollment in health insurance plans through the marketplaces, facilitated by trained navigators and application counselors. All services are open to individuals regardless of background or circumstances.

Classes include: Childbirth Preparation, Refresher Childbirth Preparation, Breastfeeding Basics, Baby Basics, Grandparenting Update: Linking the Generations, Birth Center Tours, Infant Massage, Car Seat Safety, Big Brother/Big Sister Class, and more.

Crisis team (Engagement Specialist and Crisis Counselor) provide mobile crisis response.
