The Harbour's Youth in Transition program provides up to 18 months of stable housing and supportive services through two housing models to meet youth where they are and maximize opportunities for success. Youth in Transition's supervised group living provides housing for girls ages 17-21 experiencing homelessness while they finish high school, work 10-20 hours per week (banking 65% of their earnings), and make the transition to self-sufficiency.
A supportive housing program for homeless families. Participants are provided a subsidized transitional housing unit to live in while they actively work on goals that will make them self-sufficient. Target areas include income, employment, education, transportation, child care, health and housing. The ultimate goal of the program is for families to be able to obatin and remain in affordable permanent housing.
Temporary housing for veterans for up to 90 days while they find and secure permanent housing. Currently can house 11 male veterans in Lincoln and 9 male or female veterans in Bloomington. Limited accommodations possible for families. While in the GPD program, Veterans will receive several services: - Housing intervention. - Case management. - Life skills training. - Access to either prepared meals or food pantry items. - Transportation. - Assistance with applying for VA and Social Security Benefits.
These programs provide a unique array of services that share common core principles and methods. They offer development of skills and supports the clients need to master mental health challenges while reducing the negative impact of those challenges on what matters most to young people: relationships, school success, recreational activities, health and wellness, day-to-day living and early career experiences. Services include: - Early Intervention & First-Episode Psychosis- A multistep care program that uses a multidisciplinary, evidence-informed approach to identify and treat young people at risk for serious mental health challenges or emerging psychosis symptoms. - Comprehensive Behavioral Health Services- Emerging adults with serious mental illness benefit from specialized support from people who truly understand this transitional phase as they navigate new challenges and responsibilities. - Thresholds High School- certified Therapeutic Day School for students with serious mental illnesses. The Thresholds High School provides a distinct combination of flexibility and support to help students achieve both their high school diploma and their transition goals. - Residential & Transitional Living- This program is for young adults with serious mental illnesses that require community residential services or transitional living support. - Mothers Project- Provides comprehensive assistance to mothers with mental health challenges and their children. Parents in our program are working to provide safe, healthy and nurturing environments for their children, while also receiving support with mental illness, homelessness, and/or the effects of abuse, neglect, or other traumatic experiences. This focuses on the acquisition of independent living skills, in addition to providing education and support to develop healthy parenting skills. - Homeless Youth & Families- Parents who are experiencing homelessness along with serious mental health conditions are under extreme stress. The difficulty in meeting the needs of children while attending to one’s own mental health needs is exponentially greater when the family’s housing is insecure or non-existent. The Project includes two components addressing the needs of these families. Building Flourishing Families (BFF) component for homeless pregnant and parenting youth, 16 – 23-year-olds with mental health challenges, and the PATH program for homeless families headed by a parent of any age with a serious mental health condition.

Willis House of Refuge provides supportive housing for homeless veterans, ex-offenders, and chemically dependent men.
The Transitional Housing (TH) Program provides various services as survivors strive to establish safe and stable homes free from violence. The program assists survivors with searching for affordable rental housing in communities in which survivors desire to live (based on their consideration of children's school/day-care, employment, support systems, etc.), providing rental subsidy and utility support for up to 24 months, and regaining emotional stability and health through counseling one-on-one and/or in a group setting.

Saint Leonard's Ministries provides supportive transitional housing for formerly incarcerated men and women through Saint Leonard's House, Grace House (women only), Saint Andrew's Court, and Harvest Commons Residence. This includes case management, benefits application assistance, AA groups, NA groups, relapse prevention, spiritual enrichment, and permanent housing search assistance.

Apna Ghar, Inc. (Our Home) offers transitional housing apartments provide stable, long-term housing for the families we serve so they can achieve their goals of stability and self-sufficiency. Residents are supported throughout their stay by trained staff.
Primo Center for Women and Children - Administrative Offices and Interim Housing Program 1 provides housing for homeless individuals or families for extended stays. Shelters are set up as apartments or units, and typically provide services to support for finding permanent housing and living independently.
Family Promise Chicago North Shore operates a residential program designed to move families out of chronic homelessness or housing instability towards independence and permanent housing.
Alexian Brothers - Bonaventure House is a state-licensed and funded recovery home for single men and women living with HIV/AIDS. The facility comprises 35 fully furnished private rooms, complete food service, a conference room, a TV room, a chapel, a learning center, 24/7 staffing, and administrative offices. Through the Transitional Living Program, Bonaventure House offers its residents a wide array of supportive services including case management, one-on-one and group services, peer support, recovery support, occupational therapy, and spiritual care.

Bobby E. Wright Comprehensive Behavioral Health Center - Supported Living Facility (SLF) & Community Support Residential (CSR) provides temporary and transitional housing for male and female adults, 18 years and older, who are experiencing psychiatric, emotional, situational, and environmental crises.
Casa Central offers a housing program called La Posada. The program's main goal is to restore families who are experiencing homelessness to stable and permanent living situations by providing interim housing for up to 120 days. In addition to housing, La Posada provides case management and support services to identify each family's strengths and needs, thereby addressing the root causes of homelessness and preventing recurrence. During their temporary stay at La Posada, each participant has access to on-site supportive services, including food, clothing from donations, transportation vouchers, babysitting, pre-vocational training/job readiness, ESL, and financial literacy.

A transitional living center for homeless veterans who want a chance to rebuild their lives. Many veterans enroll after undergoing intensive substance abuse treatment. Clients must undergo a screening process and abide by all rules while a part of the center. Graduates are encouraged to enroll in TLS-Aftercare, a program that offers ongoing support and crisis intervention within the community.
Case management designed for adolescents who are at risk of becoming homeless upon emancipation from DCFS. Services include assistance with locating/maintaining housing, employment, and other items necessary to sustain independence within the community.
Edward Hines, Jr. VA Hospital's Contract Emergency Residential Services program prioritizes those who are currently or imminently homeless in need of short-term transitional housing in the community.
WagiLabs is an idea incubator for elementary school kids around the world. WagiKids use curiosity, compassion, and courage to come up with ideas to promote empathy and global perspective.
Has 12 efficiency, 4 one-bedroom, and 3 two-bedroom units of Transitional and Permanent Supportive Housing for homeless adults and children. The Village provides food, shelter, clothing, supportive services, and assistance in finding employment to the best of their ability. After two months of residency, applicants who demonstrate effective use of services may be approved for transitional housing up to one year.

New Heights Apartments is a program that is open to both single survivors of domestic violence as well as survivors with children nor will all the apartments be in a single complex. NHA consists of 54 units - 18 transitional housing units (four project-based and 14 scattered-site) and 36 scattered-site rapid rehousing units. There will be 15 one-bedroom units, 23 two-bedroom units, and 16 three-bedroom units. Each client will have their own lease through the landlord.
The Interim Housing Program provides private accommodations combined with structured programming which assists clients to secure a permanent housing destination. Housing Forward operate three interim housing facilities, each tailored to specific needs of individual or families.
Respirational Development, LLC provides transitional housing for veterans during their transition to civilian life. The home offers ample room for individuals, allowing veterans to have a sense of privacy while also fostering a sense of community among residents. The home is equipped with essential amenities, including a fully functional kitchen, bathrooms, and common areas.

Through the STEPS Program, young people are provided with a stable residence, comprehensive case management, one-on-one life skills training, health care services, and other resources to help them live safe and responsible lives. STEPS houses up to eight young adults at a time. Each STEPS resident has their own bedroom and shares a bathroom with 1 other resident and a kitchen with 3 other residents. The Program can also accommodate residents' infants or toddlers and is a safe and welcoming program for pregnant and parenting homeless young mothers.
