This housing-first, trauma-informed program provides a dedicated bed, care coordination, and housing retention support.

Clustered and Scattered Site Apartments - Ignite - Belfort House & Bronzeville Youth Shelter

Ignite's independent living program is for youth between the ages of 18 to 24 who demonstrate the potential for independent living. In CaSSA, 10 youth live in their own apartments for up to two years and maintain access to all of Ignite's services. Upon program completion, Ignite either transfers the lease to the young person or helps them move into another apartment, effectively transitioning them to permanent housing.

Re-Entry Housing for Health Partnership (RHHP) - AIDS Foundation of Chicago - Main Location

AIDS Foundation of Chicago's supportive housing programs are collaborations of community-based social service providers that simultaneously care for the medical and housing needs of people who are chronically ill (with HIV or another chronic disease) and homeless. The project partners believe that when homeless individuals with chronic medical conditions are able to focus their energies on healing and maintaining their health, rather than coping with the challenging circumstances that arise from living on the streets and in shelters, their health will improve.

EMERGENCY SHELTER FOR WOMEN WITH CHILDREN | CONTACT MINISTRIES

Emergency Shelter Services - Provides emergency/crisis short-term shelter for women with children of any age, including male children (ages 13-17). The shelter provides a safe place to sleep, evening meal, laundry, and shower facilities. The length of stay is 30 days. Residents are encouraged to meet with the Emergency Shelter Lead Supervisor for long term stability plan


Transitional Shelter Services for Women with Children - Provides immediate relief to women, 18 and over, with children (male children must be 12 years of age or younger). In addition, provides life's basic needs - food, clothing, personal care items, laundry facilities, transportation, and telephones to make appointments. Supportive services provided include life skill classes, employment assistance, budgeting, family literacy, parenting classes, tutoring, and case management with a goal of finding and maintaining permanent housing.

New Heights Apartments - Family Rescue - Administration Office

New Heights Apartments is a program that is open to both single survivors of domestic violence as well as survivors with children nor will all the apartments be in a single complex. NHA consists of 54 units - 18 transitional housing units (four project-based and 14 scattered-site) and 36 scattered-site rapid rehousing units. There will be 15 one-bedroom units, 23 two-bedroom units, and 16 three-bedroom units. Each client will have their own lease through the landlord.

The L.A.M. House Transitional Home is a transitional housing facility for formerly homeless men. Applicants must agree to the L.A.M. House rules including no drug use (unless prescribed by a doctor), profanity, overnight guests, fighting, or entering rooms of other tenants. L.A.M. House is affiliated with Chicago Urban League.

Belfort House - Ignite - Belfort House & Bronzeville Youth Shelter

Belfort House is a 24-bed transitional living program for young people between the ages of 17 and 21. Youth may stay at Belfort House for up to 18 months and are engaged in productive activities including school, employment, and life skills training. With staff support, youth create Individual Action Plans which serve as road maps to help them achieve their goals while residing at Belfort House.

New Moms Housing program offers transitional and permanent supportive housing to homeless young families, providing the time and space they need to develop key life skills around health, parenting, and financial literacy and the confidence they need to build a home.Chicago Housing is for 24 months, and Oak Park is permanent supportive housing.New Moms does not take direct applications for their housing programs and does not maintain a site-based waitlist. The Harbour's Youth in Transition program provides up to 18 months of stable housing and supportive services through two housing models to meet youth where they are and maximize opportunities for success. Youth in Transition's supervised group living provides housing for girls ages 17-21 experiencing homelessness while they finish high school, work 10-20 hours per week (banking 65% of their earnings), and make the transition to self-sufficiency.
