United African Organization seeks to eliminate barriers that prevent immigrants from accessing safety net programs and connect them to resources that help achieve maximum self-sufficiency and promote their full integration in society.
Polish American Association's (PAA) Immigrant Family Resource Program is a partnership between ICIRR (Illinois Coalition for Immigrants and Refugees Rights), IDHS (Illinois Department of Human Services), and CBO (Community-Based Organizations). The purpose of is program is to inform those in the Polish community with limited English skills about services and eligibility requirements of IDHS programs. PAA offers it's clients:Bi-lingual case counseling services through one-on-one meetings with immigrants and/or immigrant families to determine their needsAssist clients in applying for/accessing appropriate programsMediate with IDHS as needed to insure receipt and maintenance of IDHS-administered benefitsProvide referrals and assistance with accessing other safety net programs for persons not eligible for IDHS-administered benefitsAssist local IDHS offices with interpretation for their Polish-speaking clients
Erie Neighborhood House provides one-on-one counseling between a therapist and a client.

HANA Center is committed to improving the quality of life and maintaining the dignity of older adults and immigrant families through support, education, empowerment, and advocacy. The HANA Center provides services for seniors that include the Silver Wing volunteer program, Education Workshops and Seminars, Housing Resource Center, Public Health Services, Public Benefits Programs including Senior Property Tax Help, and KARE (Korean-American Alzheimer's Research and Education).

Emergency and Financial Services (EFS) provides comprehensive support to people in crisis, offering services and programming to help clients increase financial literacy and reach financial stability. Interventions include financial assistance, budgeting, case management, and advocacy on behalf of our clients.
Thornton Township's General Assistance Social Worker provides support to individuals applying for Social Security Benefits and Public Aid, particularly those facing literacy and other challenges. Additionally, they assist in relocating homeless and abused individuals to safe and stable shelters, make appropriate referrals for clients dealing with substance abuse and mental health issues, and offer guidance in completing complex forms and applications. Lastly, the social worker collaborates with the Salvation Army to assist individuals who may not qualify for other assistance programs in securing a one-time emergency subsidy, distinct from ongoing general assistance.

Saint Anthony operates a community wellness program out of several locations. This includes therapeutic family support groups (available on a weekly basis), mental health services (individual, couples, and family), case management, assistance with accessing healthcare (applying for medical cards, food stamps, etc.), and free pregnancy testing.Please note that free pregnancy testing is only available at the North Lawndale and Little Village locations.

The YASS Program is a community-based case management program. The purpose of the program is to support young people experiencing homelessness around issues related to housing, mental health, substance and alcohol use, medical care, and employment. The YASS Program provides intensive individualized case management services over the course of 9 months. For these 9 months, a participant will be a part of a team of support with the goal of connecting you to long-term stable housing, employment, and other community resources.
Pilsen Food Pantry's Medical Access Closet (MAC) is open Wednesday and Friday mornings to provide pro bono physical therapy services and medical supplies to people unable to afford these services, either due to non-insurance status, therapy waiting lists, or co-payment problems. MAC also provides supplies and food for Chicago Street Medicine, a medical access program for street-based people. Since its inception, at least 500 people have received services through MAC.

AIDS Foundation of Chicago's corrections case management program supports detainees who are HIV-positive as they are released from incarceration. The program connects clients from the moment they leave jail or prison. With support from AFC and other partners, those clients receive a primary care medical appointment, housing, substance abuse services, and/or mental health services.

Covenant House Illinois provides holistic trauma-informed care to meet each person's individual and unique goals. Through their residential program, youth live at CHIL while receiving clinical case management and trauma-informed wrap-around services. Facing Forward partners with Chicago Public Schools and University of Chicago Urban Labs to provide housing-focused case management to help students and their families who are experiencing homelessness or housing instability. Our Family Empowerment Team provides additional targeted services to promote healthy development and academic success for children and youth in each household.
The experienced bilingual staff helps assess the barriers standing between asylum seekers and the care they need. HACES cares about the physical and mental needs of each individual that we serve. Our case managers work closely with new arrivals to advocate for them in an effort to create self-sufficient community members. The SBA program's mission is to listen and understand our clients' needs and to connect them to the appropriate resources and public benefits.

Saint Leonard's Ministries provides supportive transitional housing for formerly incarcerated men and women through Saint Leonard's House, Grace House (women only), Saint Andrew's Court, and Harvest Commons Residence. This includes case management, benefits application assistance, AA groups, NA groups, relapse prevention, spiritual enrichment, and permanent housing search assistance.
