Saint Anthony operates a community wellness program out of several locations. This includes therapeutic family support groups (available on a weekly basis), mental health services (individual, couples, and family), case management, assistance with accessing healthcare (applying for medical cards, food stamps, etc.), and free pregnancy testing.Please note that free pregnancy testing is only available at the North Lawndale and Little Village locations.

Through the Family Support and Community Engagement (FsACE) program eligible Suburban Cook County families are provided assistance with finding a job and advancing careers.
AIDS Foundation of Chicago's corrections case management program supports detainees who are HIV-positive as they are released from incarceration. The program connects clients from the moment they leave jail or prison. With support from AFC and other partners, those clients receive a primary care medical appointment, housing, substance abuse services, and/or mental health services.

Saint Leonard's Ministries provides supportive transitional housing for formerly incarcerated men and women through Saint Leonard's House, Grace House (women only), Saint Andrew's Court, and Harvest Commons Residence. This includes case management, benefits application assistance, AA groups, NA groups, relapse prevention, spiritual enrichment, and permanent housing search assistance.

Covenant House Illinois provides holistic trauma-informed care to meet each person's individual and unique goals. Through their residential program, youth live at CHIL while receiving clinical case management and trauma-informed wrap-around services. Clinical case managers support youth as they navigate and access community services, establish and work toward their goals, and secure permanent housing that fits their needs.

Caring, professional counselors provide confidential individual, family and group counseling to survivors of sexual violence and their non-offending family members. Services are offered to women, children and men who have been victims of assault. Case management services are available to help survivors of sexual violence and their family members identify and access community resources. Case management is a collaborative process of identifying, planning, and facilitating access to resources and services to meet the needs of individuals and families.

The Linkage to Care (LTC) team will guide you through the healthcare system to help you get the resources you need to thrive with HIV. Assistance may include accompaniment to appointments, benefit applications, behavioral health screening and referrals, medication assistance and more. The LTC program works with clients who are newly diagnosed with HIV (within the last 90 days) or those clients who have fallen out of care.
The Department of Family and Support Services (DFSS) provides city of Chicago residents with case management and public benefits enrollment through its six Community Services Centers. DFSS also funds private agencies to provide a range of support services including employment training and placement, education, substance abuse treatment referrals, mental health care, domestic violence assistance and more. Other services include provision of clothing and referrals to local food pantries, assistance with Regional Transportation Authority Ride Free Program, and assistance accessing 311's shelter intake system.
TCA Health provides various support services, including: health insurance enrollment along with assistance with enrolling in TANF, WIC & SNAP. A Care Coordinators team also helps to bridge the gap between the patient and service providers and can help facilitate appropriate services, schedule appointments, assist with community resources, and encourage health promotion through patient education.

CCAC's WIC Grocery Centers provide a variety of WIC-approved foods and formula, including specialty formulas. Shopping moms and caregivers can also access social services and nutrition education onsite. Additional services include issuance of photo ID cards, cooking classes, paternity establishment, access to tax assistance during tax season through Ladder Up, and Care Coordinators assisting with connecting with additional social services. Additional fresh fruits and vegetables from Farmers' Markets are available during growing season.All WIC Grocery locations accept the Illinois WIC EBT card. For additional WIC-eligible locations, please contact Catholic Charities or the nearest WIC Clinic office.

Heartland Human Care Services' Youth and Family Services partners with recently arrived refugee youth and their families, working together towards empowerment and self-sufficiency. They encourage respect for self and heritage, support advocacy for quality education, and foster healthy reintegration into new communities. Refugee Youth and Family Services provides a continuum of services to refugee youth including K-12 Case Management, Family Literacy, Early Childhood Education, After School programming, Substance Use Prevention Programming, Tutoring, and Youth Mentoring. During the summer, the program provides a six-week summer program.
Polish American Association's (PAA) Immigrant Family Resource Program is a partnership between ICIRR (Illinois Coalition for Immigrants and Refugees Rights), IDHS (Illinois Department of Human Services), and CBO (Community-Based Organizations). The purpose of is program is to inform those in the Polish community with limited English skills about services and eligibility requirements of IDHS programs. PAA offers it's clients:Bi-lingual case counseling services through one-on-one meetings with immigrants and/or immigrant families to determine their needsAssist clients in applying for/accessing appropriate programsMediate with IDHS as needed to insure receipt and maintenance of IDHS-administered benefitsProvide referrals and assistance with accessing other safety net programs for persons not eligible for IDHS-administered benefitsAssist local IDHS offices with interpretation for their Polish-speaking clients
Thresholds' Youth & Young Adult Services (YAYAS) offer groundbreaking programs for adolescents, young adults, families, young mothers, and their children. These programs offer the development skills and support they need to master mental health challenges while reducing the negative impact of those challenges on what matters most to young people: relationships, school success, recreational activities, health and wellness, day-to-day living, and early career experiences. Facing Forward partners with Chicago Public Schools and University of Chicago Urban Labs to provide housing-focused case management to help students and their families who are experiencing homelessness or housing instability. Our Family Empowerment Team provides additional targeted services to promote healthy development and academic success for children and youth in each household.
Equal Hope provides healthcare navigation assistance for services such as cancer screenings, mammograms, COVID vaccine scheduling, and medical home placement.

Franciscan Outreach case managers conduct initial assessments and provide guidance to individuals experiencing homelessness. Case managers also provide individuals referrals to the following services: ID documents, Social Security benefits, public aid resources, veterans benefits, primary and mental health services, substance use disorder (SUD) treatment, legal aid, job training programs, employment opportunities, immigration services, and housing programs. This location also services as an in-person Coordinated Entry System access point.Please note that case management at the Pilsen location is only offered to current guests of that shelter.

The LoSAH Center of Hope/Centro de Esperanza provides mental health care to community residents of Logan Square, Avondale, & Hermosa in Chicago. Because they are taxpayer-funded, they can only serve a specific area of the city. The LoSAH Center offers bilingual individual, couples, & family therapy to individuals with a variety of insurances, as well as uninsured individuals.

UI Health - Outpatient Care Center - Two-Generation Clinic offers social workers that work closely with doctors, nurses, and other medical professionals to help address the personal, family, and social problems that may arise from or affect one's health needs and medical care.

Chicago House provides linkage to medical care, eliminating barriers to adhering to medications and receiving other supportive wraparound services, including housing, employment, insurance, and benefit navigation to individuals newly diagnosed, those who are highly vulnerable, and those who are living with HIV.
