Provides development training and support to individuals with developmental disabilities. Services determines eligibility for state funded developmental disability services. Programs may also provide referrals, linkage to services and provides advocacy and monitoring of individuals with developmental disabilities. Programs include prioritization for urgency of need for services (PUNS), preadmission screening (PAS), and individual service and support advocacy (ISSA).

Offers help with water bills and prescription drugs, up to $50. May be used twice in a 12 month period.

Connects individuals and their faith community to needed healthcare and wellness support. Program provides access to education, screenings, and other healthcare resources.

Services include:

-- Health screenings (blood pressure, medication reviews, body mass index, and more)

-- Health promotion and education programs that cover topics like chronic disease, advance care planning, fitness, nutrition, and CPR

-- Assistance with navigating healthcare systems

-- Assistance with connecting to healthcare resources, community organizations, and other supportive programs

Circuit Court handles district court records such ass traffic violations, probate, juvenile and small claims. Juvenile Court service handles intake/probation/supervision. Also handles Child support assistance, protective orders.

Created so that eligible children and adults who have a disability could obtain legal advice and representation to protect and enforce their rights guaranteed by Illinois' mental health laws. Attorneys provide needed assistance to persons with disabilities in a variety of settings:

- Mental health facilities.

- Residential programs.

- Community placements.

- Nursing homes.

Issues addressed include, but are not limited to: admission and discharge from hospitalization, adequate treatment, refusal of unwanted services, and confidentiality of mental health records. Assistance takes the form of information, referral, and legal representation in court hearings and administrative proceedings.

Environmental health services include:

• Private sewage disposal & water supply programs
• Food safety
• Food service sanitation program
• Vector control

Offers after school programs at Lewis Lemon Community Center (LLCC) and Washington Park Community Center (WPCC).

- LLCC After School Program: Enrichment program offered to Lewis Lemon students. Activities include homework help, board games, arts and crafts, gaming systems, and the opportunity to participate in organized basketball and football teams. A daily snack is provided. Students also have the opportunity to participate in the 1st Century Program, which offers help in reading, math, and science. The students of this program must be teacher recommended.

- LLCC Teen Evening Program: Offers open gym, gaming systems, and more. Staff keeps teens active and safe.

- WPCC After School Program: Provides an after school program for youth. A snack and boxed dinner are provided.

Activities include:

- Homework help.

- Arts and crafts.

- Gymnasium games.

- Fitness.

- Computer lab.

- Outdoor play (weather permitting).

- Cub Scouts.

- Girl Scouts.

- Sports teams.

WPCC Teen/Tween Evening Program

Provides a community institution for families dealing with child abuse, domestic abuse, and elder abuse. Services include education and prevention, contributes to the improvement of the legal system and the administration of justice.

Offers a free home assessment, which will install energy-saving products, such as programmable thermostats, ENERGY STAR® certified LEDs, WaterSense® certified showerheads, WaterSense® certified faucet aerators, hot water pipe insulation, air sealing and insulation, furnace tune-ups.

Provides sexual health education. Testing and treatment for chlamydia and gonorrhea.

Provides activities and enrichment programs for children and teens.

Provides reproductive and contraceptive services through on-site clinics, offering annual reproductive examinations, education on reproductive health and contraceptive methods, and tailored contraceptive options based on individual needs. Follow-up laboratory reports and infertility services are available, including referrals for individuals seeking to achieve pregnancy. 

Pregnancy testing is provided, and males can access certain services through this program. Additionally, the program incorporates communicable disease control measures, including tracking and preventing the spread of diseases through surveillance, reporting, and appropriate interventions such as isolation and contact tracing. Screening for sexually transmitted infections, including gonorrhea, syphilis, genital herpes, and other infections, is conducted using visual examinations, blood tests, and sample analyses to ensure early diagnosis and treatment.

Assists older persons or their caregivers in identifying their needs and place them in contact with appropriate community resources or service providers.

Follow up activities are included such as making sure the older person was able to receive the service or help the older person obtain the service.

Provides hospice and palliative services to veterans with life-threatening illnesses. Services focus on comfort, quality of life and reducing suffering.

Source of factual medical information relating to symptoms, risks, how to be tested, etc.

Provides transportation to participants, ages 60 and over, as well as individuals ages 18 and over with a disability, living in Sangamon County. Vehicles are wheelchair accessible. Reservations are required.

Provides Head Start and Preschool for All programs, and family engagement activities.

Offers nutritious home cooked meals provided by Golden Meals are available to help promote healthy eating and reduce isolation by encouraging fellowship. Many social activities (movies, bingo, friendly Mexican dominoes, card games, knitting/crocheting, pool, etc.) are offered before and after the meal on selected days of the week.
