Offers internal medicine, pediatrics, general health care services, as well as ancillary services including laboratory, x-ray, pharmacy, nutrition, mental health counseling, health education and translation services.

Provides treatment for substance use disorders. Offers counseling, group therapy, or medication based on the patient's specific needs. The Opioid Addiction Treatment Program (OATP) provides specialized treatment services for opioid addiction and MAT services (medication assisted treatment).

Age-appropriate educational programs are available to students from preschool through college to help raise awareness of sexual violence. Prevention programs are also available to community members interested in learning more about sexual violence. Programs are tailored to meet audience needs.

Professional trainings are also available to service providers and professionals to increase knowledge of sexual violence and raise awareness of community resources. Programs topics include personal body safety, sexual harassment, sexual assault, date rape drugs, and teen dating violence.

Provides services to students to help them become successful learners and remain at grade level with age peers. Services are intended to improve personal self-esteem/worth through guided learning and a personal tutor. Scholarships are available through the LCCCY.

Provides crisis intervention for youth who have run away, who are locked out, or experiencing homelessness.

Services:

- Offers emergency psychiatric or substance abuse treatment if needed

- Follow-up care plan for youths reunited with family

- Short-term or long-term placements if reunification is not immediately possible

Offers mental health 24-hour assessment, partial hospitalization, psychiatric outpatient treatment programs. Services may include psychiatric care, group and individual therapy, medication, and case management. Inpatient programs offer psychiatric inpatient units for those in need of extra support and case management.

Provides limited, one-time financial assistance in the areas of food, dentures, eyeglasses, medicine, energy bills, and more.

Provides the following services by a licensed professional counselor:

- Mental health screenings and assessments

• Recovery planning
• Individual counseling and therapy
• Case management coordination
• Continuing recovery planning
• Ongoing support for individuals with a mental health need
• Limited psychiatric services by consulting psychiatrist

Provides in person behavioral health crisis services to youth and adults. Services include a mental health assessment, crisis intervention services, such as focused support, de-escalation, stabilization, development of a safety plan, and linkage to additional outpatient services. If psychiatric hospitalization is needed staff will assist with this process as well.

Provides a food pantry for those in need. USDA government commodities are distributed along with purchased and donated food, including weekly donations of perishable items from Trader Joes.

Offers a weight management program to veterans. Program provides weight-loss support groups to assist in weight loss, healthy eating tips and cooking classes, behavioral change strategies to assist in living a healthier lifestyle.

Provides government services to the residents of Pontiac.

Provides a Court Appointed Special Advocate (CASA). A CASA volunteer is a trained community member who is appointed by the Juvenile Court Judge to represent the best interests of abused and neglected children who are brought before the court. A volunteer has three main roles

- To serve as fact finder for the judge (by handling only one or two cases, the volunteer has time to thoroughly explore the history of each case).

- To speak for the child in the courtroom, advocating for the child’s best interests.

- To act as a “watchdog” for the child during the life of the case, ensuring that the case is brought to a swift and appropriate conclusion.

Provides various behavioral health services to individuals with disabilities. Behavioral health services include:

- Psychiatric evaluation.

- Medication management.

- 24/7 crisis intervention.

- Coordinated Case Management.

- Outpatient individual and group therapy services.

- Substance use treatment for co-occurring disorders.

Provides transportation to any person 60 years or older with pick-up site in Logan or Mason County. Transportation Services can be obtained for regular trips to medical appointments or treatment. Trips must be scheduled 24 hours in advance.

Please note: Persons under the age of 60 can utilize transportation but must pay the full fare (suggested donation). Persons over the age of 60 take priority.
