Chicago Public Schools operates Student Outreach and Re-Engagement (SOAR) Centers that help link youth and families with the support needed to address the fundamental issues driving lack of engagement in school. The program has four citywide SOAR offices in neighborhoods with the highest concentrations of out-of-school youth.
Offers educational opportunities and resources to service members, veterans, and their families. Trainings include: - Mental health and PTSD awareness - Military culture teach-ins - School outreach to share military/veteran history - Other speaking/educational engagements as requested
Mobile outreach vehicle where people can exchange used needles for sterile needles to reduce transmission. Also offers peer support meetings, education, and treatment navigation. "Look for the big purple truck!" For those in need of safe supplies (including needle and syringe exchange), please do the following: 1. Text or call 224-297-4393 2. Tell current location, including address (or the name of a building/store/service nearest) city and zip code, and the types of supplies needed 3. At that time, an outreach team member will attempt to schedule a phone or web call for harm reduction education and counseling. NOTE: This step is not mandatory to receive supplies. 4. An outreach team member will put together a supply request using gloves and other safety precautions. 5. A member of the team will coordinate a drop off, leaving the bag of supplies at the requested location. They will text when the supplies are delivered.**

Teen Outreach Program (TOP) empowers teens with the tools and opportunities to develop social-emotional skills, build healthy relationships and community connections, develop a sense of purpose, and avoid risky behaviors. TOP promotes positive youth development through engaging curriculum, community service learning, and supportive relationships with adults. The Teen Connection Project (TCP) is a 12-14 week program designed to improve peer relationships, social-emotional skills, school belonging/engagement, and well-being among high school youth.

Veterans Justice Outreach Program: Provides outreach to veterans in contact with the justice system through encounters with police, jails and courts. The Veterans Justice Outreach social worker provides support to veterans involved in the Veterans Court, a court diversion program for veterans.
Individuals seeking assistance can ensure that they will be treated with respect and dignity, taking their culture and linguistics into consideration. In addition, individuals may also be connected or receive information about resources within the community.
Community Outreach Program: Provides community outreach to increase level of accountability of high risk juveniles allowed to return to the community and to heighten the visibility of juvenile officers. This is done by juvenile officers accompanying Festus police officers on rounds and conducting unannounced visits to the homes of juveniles.
Corazón's Department of Health Services specializes in delivering sexual health care services in a non-judgmental, sex-positive, and LBGTQ+ affirming setting. They offer confidential testing and counseling for HIV, gonorrhea, and chlamydia Monday through Friday (walk-ins always available) and supply free condoms and safer sex kits. They also provide referrals for other client needs, such as mental health, domestic violence, and much more. In support of the community's youth, they partner with Morton East High School in Cicero and Heritage Middle School in Berwyn to offer classes on sexual health and teen pregnancy prevention.

BUILD's Mental Health Program is designed to increase access to mental health care and substance abuse treatment for young people impacted by these issues. The Clinical Wellness teams offer onsite and school-based counseling, family services, art therapy, restorative justice programs and more. Counselors and therapists integrate their sessions with other BUILD programs, working to eliminate the stigma often associated with mental health care.

Live4Lali operates a mobile truck as well as a shipping service that provides drug use harm reduction supplies, such as naloxone (NARCAN) and fentanyl test strips. In-person distribution is also availabe at their main office in Arlington Heights.
The Chicago Department of Transportation's (CDOT's) SAFE Ambassadors make up the outreach and engagement team. SAFE stands for Streets Are For Everybody. Since 2001, the Ambassadors have provided direct education and encouragement to Chicagoans regarding traffic safety, active transportation options, and multiple CDOT projects.
La Casa Norte's Public Health Ambassadors youth outreach program works to curb health inequities in Chicago. Based at the Community Assistance Office, youth ages 16 through 24 earn a stipend while training to be advocates for their neighborhoods. Youth ambassadors help their communities access vaccines and other resources, including rental and internet assistance.

Outreach Program: Provides support throughout the recovery process from treatment to successful independence by identifying the mental and physical health needs of each person and offering links to community service programs.
Offers speech, listening and language therapy to children birth through high school in the convenience of their home or school.
Outreach: Provides interventions initiated by an agency or organization for the purpose of identifying potential clients or their caregivers and encouraging their use of existing services and benefits.
- Adult MSM Program: Provides social and support groups for both HIV-negative and HIV-positive men. Also, offers HIV/STD testing, bar/bath house outreach and education, social marketing campaigns and LIFE, and education health-promoting program for HIV-positive men. - Mobile Outreach Program: Provides HIV/STD education, testing and linkage to care services for people who are newly diagnosed HIV-positive and outreach for high-risk individuals.
The Love and Friendship Adoption Center provides fostering and adoption services for dogs and cats. The Center also provides education on the humane treatment of animals through public events. The center welcomes volunteers and relies on donations to continue to provide services. Lower cost micro-chipping and registration services are also available at the center.
