BUILD's Mental Health Program is designed to increase access to mental health care and substance abuse treatment for young people impacted by these issues. The Clinical Wellness teams offer onsite and school-based counseling, family services, art therapy, restorative justice programs and more. Counselors and therapists integrate their sessions with other BUILD programs, working to eliminate the stigma often associated with mental health care.

Hamdard Health Alliance works in close collaboration with our partners to develop culturally relevant health programming. Health Educators work alongside Primary Care Providers to provide the tools, knowledge, and resources individuals need to take control of their health challenges. Hamdard's Outreach and Education department manages all types of community initiatives to ensure that our patient population has the information necessary to make good choices about their health and wellness.

Provides the following services for older adults, adults with disabilities, and caregivers: - Linkage to available benefits and services. - Community outreach, providing information and assistance in gaining access to benefits and services in the homes of older adults. - Senior Health Insurance Program (SHIP): Provides Medicare counseling, information and assistance and enrollment assistance. - Senior Health Assistance Program (SHAP): Provides assistance with benefit applications such as SNAP (Food Stamps), Low Income Subsidy/Extra Help, Medicare Savings Program, and Benefit Access. - Family Caregiver Support: Provides information, assistance, and referrals for individuals caring for a family member. - Options Counseling: Assistance for adults and caregivers through an independent decision support process to determine community based care choices. - Recreation programs.

La Casa Norte's Public Health Ambassadors youth outreach program works to curb health inequities in Chicago. Based at the Community Assistance Office, youth ages 16 through 24 earn a stipend while training to be advocates for their neighborhoods. Youth ambassadors help their communities access vaccines and other resources, including rental and internet assistance.
Directs individuals and/or their family members to available services and community resources which meet the individual's expressed need, such as enrollment activities related to the pharmaceutical assistance programs such as Medicare Part D.

Breakthrough provides a long-term, comprehensive approach to reducing violence in Chicago for adolescents and adults ages 13+. Through prevention, intervention, and restoration, Breakthrough's Violence Prevention team works year-round to increase hope in the community by creating opportunities for residents most at risk of violence to pursue a safe, stable and engaged East Garfield Park.
Corazón's Department of Health Services specializes in delivering sexual health care services in a non-judgmental, sex-positive, and LBGTQ+ affirming setting. They offer confidential testing and counseling for HIV, gonorrhea, and chlamydia Monday through Friday (walk-ins always available) and supply free condoms and safer sex kits. They also provide referrals for other client needs, such as mental health, domestic violence, and much more. In support of the community's youth, they partner with Morton East High School in Cicero and Heritage Middle School in Berwyn to offer classes on sexual health and teen pregnancy prevention.

The SAFE Ambassadors travel all over the city educating people of all ages and backgrounds about pedestrian and bicycling safety. The Ambassadors offer presentations, safety literature, and bicycling activities at parks, schools, on-street events, and other locations. The Ambassadors can also provide tools for seniors to navigate 311, reach out to elected officials, access transportation resources, and select safe routes through their neighborhoods.
Provides intensive behavioral health treatment to adults with severe mental illness. Offers services in a community-based setting by an team of mental health professionals. Programs range from one of three levels of treatment intensity: - The Assertive Community Treatment - The less intensive Community Support - The Community Support Individual. Considered the final level of service before full self-sufficiency. Offers a multidisciplinary approach of intensive services provided where and when needed, 24 hours a day, seven days a week. Teams include peer support specialists and practitioners with expertise in: - Psychiatry - Nursing - Social work - Substance use treatment - Employment These teams initiate: - A housing search - Developing a care plan - Continuing to work with clients over the long-term.
Offers Senior Information Service (SIS) Outreach & Referral. Assistance is provided in our office, over the phone, during home visits by our Outreach SIS workers at our workshops held throughout Iroquois County. Assistance includes: - Answer all questions including qualification questions. - Assess clients' needs and make client referrals. - Help clients fill out and file application/documents required for various programs such as Benefit Access Application, P-Tax form, Senior Citizen Homestead Tax Freeze form, Medicare Part D and pharmaceutical programs, Medicaid/Snap applications, Social Security Extra Help program, and AARP Income Tax (only low income, times and days will vary each year). - Advocate on behalf of clients to assure access to needed services.
